BBH System of Care (SOC) Data Collection System

Background

On behalf of BBH, West Virginia University’s (WVU) Office of Health Affairs has implemented a Database Collection and Management System utilizing Epi Info. The West Virginia System of Care Evaluation Database Management System will be used to collect and securely store sensitive health information from children and families receiving services from the Bureau for Behavioral Health's System of Care grant recipients.

Epi Info replaced the Excel reporting forms used by System of Care grant recipients effective August 2, 2021. Grant recipients are required to use Epi Info reporting forms beginning on that date.

Download and Installation

In order to perform data entry and submission, System of Care providers will need to download and install the following software components:

BBH SOC Installation Guide (updated 9/28/2023 version 2.0). Please open and review this guide. It includes detailed instructions on how to download, install, and configure the software components below.
CDC Epi Info. Epi Info is a data collection tool developed by the Centers for Disease Control and Prevention (CDC). BBH has developed a modified version of the software to better facilitate installation and data transfer. The installation files for this version of Epi Info can be downloaded from the link above. Refer to the guide above for installation steps.
BBH SOC Epi Info Project Files (updated 9/27/2023 version 2.0). Once you have installed Epi Info and SQL Server, the files in this package are required to run the Epi Info data entry forms. Please follow the instructions in the installation guide regarding how to extract, save and run files in this package.
Migration Tool. This tool is used to migrate data from BBHSOC Epi Info Version 1 to BBHSOC Epi Info Version 2. Please see the installation guide for instructions.
